Why do few hair transplant clinics perform procedures that require over 3,000 grafts?

Many hair transplant surgeons claim that high graft number procedures – those requiring over 3,000 FU grafts - are not commonly performed in their clinics due, in part, to time constraints but also the availability of suitable FUE grafts in the donor sites. Others go as far as to cite such surgeries as ill-advised due to the impact on blood supply and hair survival from the placement of high numbers of grafts.
 
One publication used to defend this position is Hair Transplantation: Current Concepts and Techniques by Walter P. Unger, published in 2005, in which the author believes,
 
“… the recent trend of transplanting 3000 or more FU per session, by some practitioners, should be viewed with some scepticism with regard to hair survival. 
 
For example, although each 18-G needle hole may appear small, and typically produces an incision that is “only” approximately 1.2mm long, 3,000 of these incisions will produce a total of 360cm of incisions in an area that is often no larger than the palm of an average man's palm.”
 
Dr Tam is keen to draw your attention to where the author refers to implantation using an 18-gauge needle that creates 1.2mm long incisions; this is a somewhat outdated practice. Hair transplant surgeons generally agree it is safe to make 45 incisions using a 1mm blade within a 1cm2 zone without compromising the blood supply. The skin should heal adequately, and grafts should grow despite a combined 4.5cm injury within the 1cm2 area. This of course limits them to 45 grafts per 1cm2.
 
However, what makes Dr Mark Tam’s approach using the IDEAL technique different and a long way away from the advisory caution in this published paper is that unlike most, he uses custom-made blades that create 0.8mm incisions. Smaller blades reduce injury due to smaller incisions and allow for higher density placement of more grafts (50 per 1cm2) without incurring more injury (creating a combined 4cm) or adversely affecting hair survival.
 

3,000 to 4,000 graft transplants are possible with custom-made surgical tools

Looking again at the clinical paper often cited to denounce high graft number FUE hair transplants, the author discusses his concerns with so-called “dense packing” and states,
 
“… FU densities of 30, 40, 50, or more per cm2 (respectively 3.6, 4.8, and 6.0 cm of incisions in each cm2 of recipient area) should sound alarm bells until satisfactory hair survival studies are produced by the practitioners who advocate these approaches.”
 
He suggests that the placement of 50 grafts within a 1cm2 area would create a 6cm injury. Remember, this publication assumes the creation of 1.2mm incisions for each graft placement, not the 0.8mm incisions performed by Dr Tam.
 
Dr Tam has honed his skills and expertise over several decades in multiple clinical environments, treating many complex hair transplant cases to become a craftsman of hair restoration. Custom-cutting surgical blades and using smaller FUE punches cause less injury than conventional methods discussed in textbooks and published reviews, reduce damage and scarring at both the implantation and donor sites, impact less on blood supply and graft survival, and achieve a denser result.
 

Will I have enough hair for a hair transplant using over 3,000 FUE grafts?

Donor areas at the back and side of the head are a precious resource; they must be respectfully and carefully managed. You might ask, how is it possible to use more grafts? Won’t I run out of hair?
 
Again, this is a common reason given by many hair transplant clinics for avoiding doing high-graft-quantity hair transplant procedures, stating that there is a finite amount of hair available in the donor sites – especially of the right kind of hair grafts, such as single hair follicles required for the hairline.  They believe this constricts how many FU grafts are available to transplant in the first instance and will deem the procedure non-viable.
 
Certainly, this is true to an extent; to achieve a natural restoration of the hairline and temple points and produce a superior, long-lasting hair transplant result, we require a significant quantity of single-hair follicles that are more prevalent along the natural hairline. However, most available hair in the donor areas is multi-hair follicles, with limited single-hair follicles. To secure more hair grafts of the 'right type' to create a natural-looking result, each patient will require individually adapted graft harvesting and placement to gather the best and most appropriate grafts. The key to deeming a procedure viable is knowing how to optimally choose and harvest the FU grafts or modify them to suit.

By selectively separating hair follicles, the hair graft number or yield secured from the donor site(s) is higher but with reduced intervention, allowing Dr Tam to achieve higher density placement, using 50 grafts per cm2 with over 3,000 grafts, where required. Therefore, the real bonus from the regenerative methods employed with the IDEAL technique over conventional methods that use larger surgical tools is that fewer follicular units are taken from the donor area to achieve the goal, just like preserving a well-managed forest for future use by carefully thinning out the saplings.

How can I tell how rapidly or severely my hairline is receding?
 
Not everyone who suffers from hair loss caused by male pattern baldness loses their hair at the same rate or to the same extent – but some can experience extreme hairline receding and may wish to seek intervention promptly with anti-hair loss medication or hair transplantation.
 
The simplest way to judge what is happening is by documenting and measuring, which can be repeated regularly, around every three months, to build up a record of hair loss. Check the distance from the top of your nose to the centre of your hairline and again to your temples and keep a note of any changes in millimetres. Taking regular photographs in the same position is also helpful.
 
What is a high graft quantity FUE hair transplant?
 
In cases of extreme hairline receding, it is not uncommon for a restorative transplant procedure to require over 3,000 follicular unit grafts to achieve a satisfying and optimal result in terms of the thickness or density of the transplanted hair and a normal-looking hairline. Some patients may need up to 4,000 grafts, considered a very high quantity.
 
What is uncommon is finding a hair transplant surgeon or clinic that can accommodate and offer procedures to achieve this in a single surgery, in a single day. How do I work out how many hair grafts I will need?
 
Hair transplant surgeons often use a recognised measurement pattern called the Norwood Scale to measure the severity and extent of hair loss across the whole head, including the hairline, to determine graft numbers. Although, this is a guesstimate at best. Dr Mark Tam prefers to employ more accurate calculations to quantify graft numbers using his Treatment Area Mapping and Measurements (TAM+M) process. 
 
Rather than relying on generalised hair loss patterns, zones, and scales, he can take personalised, detailed measurements and drawings from the affected zones on your scalp to accurately calculate your total treatment area. Alongside hair density parameters and the primary hair follicle types required, depending on the severity of hair loss at the hairline and temple points, Dr Tam can determine the exact graft numbers to harvest from the donor sites. How do you carry out 3,000+ graft FUE hair transplants in a single surgery?
 
The use of pioneering harvesting, preparation, and transplantation methods as part of the IDEAL technique, including individually separated hair follicles and partial follicular unit extraction, optimises graft numbers (to include single, double, and triple hair grafts) whilst protecting the donor site(s) and minimising scarring. You can continue having a short haircut at the back and sides post-transplant because signs of intervention are barely noticeable, even with high graft number transplants. Plus, preservation of the donor site with regenerative extraction methods, even for a high graft number operations, means we can consider using the areas again for future procedures to address additional or continued hair loss over time.
 
When treating significant hairline receding that naturally requires a high quantity of grafts to achieve a natural, ultra-high-density result, the combination of optimal graft creation from the available donor resource and hours in the day leads to success. This is time-consuming work, hence the large technical team that supports Dr Tam as he operates. Most private hair transplant clinics cannot dedicate longer than an 8-hour day to a single-patient surgery because of overheads and staffing. This minimises your downtime, time away from work and home, plus travel and overnight expenses. It also means the outcome is achieved in a single pass, rather than in incremental stages or at reduced density. There will be no need to wait a year for the results from the first procedure before embarking on a second.
 
Staggered procedures can leave you feeling somewhat ‘half-done’. Similarly, using fewer grafts in the first procedure and hoping to do a second pass in the future is counterintuitive because the second attempt will necessitate leaving increased margins from the previously placed grafts to avoid damage, meaning density from two passes in the same area will never be as optimal or as high as when done in the initial, single pass.
